Can FMS be used to predict injury in Extreme Training Programmes?

Therapy Expo 2018

Well, Therapy Expo was amazing.

It was great to catch up with old friends and meet new people in the Industry.
RocDoc Alfie, Paul from Pulseroll, RocDocRob, The Endurance Physio, Rehab Guru and Inside Edge Physiotherapy were all on great form.

I was presenting both my research and Audit on Extreme Training Programmes and Functional Movement Screening.
We cannot predict injury, but we can potentially identify vulnerability.

Both Uzo and Dale were presenting to the Therapy Crowds.

Therapy Expo the Bulletproofbodies Team salute you.

Do you have Movement Literacy?

Movement literacy is about knowing how to read your environment through the medium of your body.

This is also referred to as Physical literacy.

Physical Literacy is the mastering of fundamental movement / sport skills that permit a child to read their environment and make appropriate decisions, allowing them to move confidently and with control in a wide range of physical activity situations.

Physically literate individuals consistently develop the motivation and ability to understand, communicate, apply, and analyse different forms of movement.

They are able to demonstrate a variety of movements confidently, competently, creatively and strategically across a wide range of health-related physical activities.

These skills enable individuals to make healthy, active choices that are both beneficial to and respectful of their whole self, others, and their environment.

Functional Movement Screening for Injury Surveillance

I started off my Functional Movement journey whilst I was still serving as a Physiotherapist in the Royal Army Medical Corps.

Working with the Military elite we used FMS as an early warning detection system as part of a comprehensive injury surveillance system that was proactive rather than reactive.

The FMS involves seven movement challenges that represent normal functional movements:

1. Deep Squat
2. Hurdle Step
3. In-Line Lunge
4. Shoulder Mobility
5. Active Straight Leg Raise
6. Trunk Stability Push Up
7. Rotatory Stability

The overhead squat is used to determine range of movement in the shoulders, thoracic spine, hips, knees and ankles. It also tests motor control in symmetrical stance.

The Hurdle Step is designed to test range of movement and motor control in

single leg stance with weight transfer over an obstacle

The In-Line Lunge tests your ability to stabilise in an asymmetrical foot stance.

This test is more challenging to stay balanced in -line and maintain an upright torso.

The FMS gives you a score out of 21;

0 = Pain

1 = Dysfunctional Movement

2 = Normal Movement

3 = Optimal Movement

The top score of the day was by a mature athlete who was inappropriately dressed and still made it look easy!

Triple Marathon Challenge - Claire Chambers Capital Radio

It was great to help Claire Chambers from Capital Radio East Midlands on her extreme challenge of running three marathons in three days.

Claire will run the marathon distance of 26.2 miles on three consecutive days in April:

Wednesday 5th - Derby
Thursday 6th   - Leicester
Friday 7th – Nottingham

You can keep track of Claire’s progress and make a donation £5 by texting RUN to 70766.
All donations go to Global’s “Make Some Noise.”

Claire & Tom from Capital Radio Midlands came to see me at the University of Salford for injury screening and prevention.

As three marathons in three days is a significant load on the body, Claire must make sure she prepares well and identify any potential problems before they occur.

Claire has already run two Marathons before in previous years so she knows what the challenge entails.

Tight hip flexors and Quadriceps are normal when performing any endurance running.

That's where I come in.

As an injury prevention specialist we try to spot asymmetries and motor control issues before the event and address these early.

My motto is simple, Prevention is better than cure.

But how do we know if we are likely to get injured?

I use the Functional Movement Screening.

The movement screen makes the invisible visible, by asking Claire to perform seven movement challenges.

The first test is the Over Head Deep Squat

The next test is the hurdle step.

Each test has a maximum score out of three:

0 = Painful Movement

1 = Dysfunctional Movement

2 = Normal Movement

3 = Optimal Movement

Claire & Tom, the Bulletproofbodies Team Salute you!

Bulletproofbodies recommends (Self) Functional Movement Screening

In 2009 Gray Cook came to the UK and presented about FMS. I thought his presentation was amazing and I could see exactly where he was going with his thought process.

Later that year I would be using FMS on a daily basis and for the forseeable future.

I then got certified to FMS Level 1 & 2 with Lee Burton teaching us on the course.

As a Physiotherapist I could use the FMS as a tool to be pro-active rather than re-active with my patients and clients.

It was a way to rank and rate movement.

It was a way to make the invisible, vissible by putting the participant through seven movement challenges.

This tool is validated in the literature for nearly 19 years!

Although there has been some critism about whether FMS or indeed any form of screening can predict injury,

I believe that this is still the best tool for contributing in the paradigm shift away from reactive medicine to preventitive self-management.

Gray Cook and Lee Burton I salute you - Awesome work!
